See our Service Area Home The 4 Levels of Hospice Care Routine Home Care Care provided at home by the family or other caregiver, assisted by the hospice staff. This is the most common level of hospice care. With this type of care, an individual has elected to receive hospice care at their residence, which can include a private residence, assisted living facility or nursing facility Respite Care Short-term care available to provide temporary relief to the patient’s primary caregiver. Respite care can be provided in a Medicare approved facility which is a hospital, hospice facility, or a long term care facility that has sufficient nursing personnel present twenty-four hours on all shifts to guarantee that patient’s needs are met. Respite care is provided for a maximum of 5 consecutive days. Care and services are established through a contractual agreement General Inpatient Care Short-term care provided for pain control or other acute symptom management that cannot feasibly be provided in any other setting. Inpatient Care begins when other efforts to manage symptoms have been ineffective. Inpatient Care can be provided in a Medicare certified hospital, hospice inpatient facility, or nursing facility that has a RN available twenty-four hours a day to provide direct patient care. Continuous Home Care Care is provided between 8 and 24 hours a day to manage pain and other acute medical symptoms. The care provided must be predominately nursing care, supplemented with caregiver and hospice aide services. This level of care is intended to maintain the terminally ill patient at home during a pain or symptom crisis. Understanding Common Changes As illness progresses, families often notice changes that can be normal parts of the end of life journey. Increased Weakness Daily activities often become more difficult. Changes in Appetite It is common for patients to eat and drink less and even stop intake as the body naturally slows down. Increased Sleeping Many patients spend more time resting and sleeping. The person may appear to be incommunicative and unresponsive. This normal change is partly due to changes in body chemistry. Sit with your loved one, hold his or her hand, do not shake or speak loudly, but speak softly and naturally. Do not talk about the person in the person’s presence. You can speak to him or her directly as you normally would, even though there may be no response. Changes in Breathing Breathing patterns may change and can sometimes become irregular. The person’s regular characteristic breathing pattern may change with the onset of a different breathing pace which alternates with periods of no breathing. This is called the “Cheyne-Stokes” syndrome. It is very common and indicates decrease in circulation in the internal organs. Elevating the head, holding hands and speaking gently may help bring comfort. Incontinence The person may lose control of urine and/or bowel matter as the muscles in that area begin to relax. Discuss with your hospice nurse what can be done to protect the bed and keep you loved one clean and comfortable. If you are unsure whether something is normal, please call us. Hospice is a health care benefit for people with life-limiting illnesses. Hospice does not provide aggressive or curative care related to the life-limiting illness. It is provided in the individual’s place of residence by a team that consists of nurses, social workers, hospice aides, volunteers, spiritual, bereavement and dietary counselors and other therapists (physical, occupational and speech) as needed under the direction of a physician. If the person does not have a primary physician, the medical director of Wings Over Texas Hospice will fulfill that role. The goal of hospice care is to control pain and other symptoms, rather than to cure the person’s illness. Hospice provides support for the person who is ill as well as his or her loved ones. Home Who chooses the hospice provider? Choosing to go on hospice and who to use as a hospice provider are personal decisions. Many patients consult with family, and medical providers. Does hospice make death come sooner? No, hospice provides its presence and specialized knowledge during the dying process but does nothing to either speed up or slow down the dying process.
