Understanding Common Changes As illness progresses, families often notice changes that can be normal parts of the end of life journey. Increased Weakness Daily activities often become more difficult. Changes in Appetite It is common for patients to eat and drink less and even stop intake as the body naturally slows down. Increased Sleeping Many patients spend more time resting and sleeping. The person may appear to be incommunicative and unresponsive. This normal change is partly due to changes in body chemistry. Sit with your loved one, hold his or her hand, do not shake or speak loudly, but speak softly and naturally. Do not talk about the person in the person’s presence. You can speak to him or her directly as you normally would, even though there may be no response. Changes in Breathing Breathing patterns may change and can sometimes become irregular. The person’s regular characteristic breathing pattern may change with the onset of a different breathing pace which alternates with periods of no breathing. This is called the “Cheyne-Stokes” syndrome. It is very common and indicates decrease in circulation in the internal organs. Elevating the head, holding hands and speaking gently may help bring comfort. Incontinence The person may lose control of urine and/or bowel matter as the muscles in that area begin to relax. Discuss with your hospice nurse what can be done to protect the bed and keep you loved one clean and comfortable. If you are unsure whether something is normal, please call us. We are here to guide you. When Should I Call Hospice? 📞Call Our 24-Hour On-Call Nurse: 817-774- 9154 Please call us anytime if your need: • Medication Refills • Supply Needs If our hospice patient experiences: Any changes that worry you • Unresolved pain or symptoms • Urinary difficulties • Unresolved Shortness of Breath • Unresolved Bowel Issues • Severe Bleeding • Any Falls or injuries • Nausea or Vomiting • Temperature of 101°F or above Please call us first: Before calling 911 • Before going to any tests or procedures Something worries you about the hospice journey • Coping Crisis • Death of Loved One You do not need to wait until morning or until the next scheduled visit. If you need bereavement support, please contact us at any time. 817-774-9154 How Do you React When Someone Dies? Because grief can be so painful, and seem overwhelming, it frightens us. Many people worry if they are grieving in the “right” way, and wonder if the feelings they have are normal. Most People Who Suffer a Loss Experience One or More of the Following: • Feel in a state of shock • Feel weak • Have a strong emotional release (i.e., crying, sobbing) • Feel tightness in the throat or heaviness in the chest • Have an empty feeling in their stomach and lose their appetite • Feel restless and look for activity but find it difficult to concentrate • Feel as though the loss isn’t real, that it didn’t actually happen • Wander aimlessly and forget, or don’t finish things they’ve started to do around the house or at work • Have difficulty sleeping and dream of their family member frequently • Assume mannerisms or traits of their family member • Feel guilty or angry over things that happened or didn’t happen in the relationship with the deceased • Fell intensely angry at the family member for leaving them • Need to tell (and re-tell) and remember things about the family member and the experience of their death • Feel mood changes over the slightest things • Cry at unexpected times • Hope returns – gradually • Begin readjustment/redefining

Call Now Understanding the 4 Levels of Hospice Care Routine Home Care Care provided at home by the family or other caregiver, assisted by the hospice staff. This is the most common level of hospice care. With this type of care, an individual has elected to receive hospice care at their residence, which can include a private residence, assisted living facility or nursing facility Respite Care Short-term care available to provide temporary relief to the patient’s primary caregiver. Respite care can be provided in a Medicare approved facility which is a hospital, hospice facility, or a long term care facility that has sufficient nursing personnel present twenty-four hours on all shifts to guarantee that patient’s needs are met. Respite care is provided for a maximum of 5 consecutive days. Care and services are established through a contractual agreement Continuous Home Care Care is provided between 8 and 24 hours a day to manage pain and other acute medical symptoms. The care provided must be predominately nursing care, supplemented with caregiver and hospice aide services. This level of care is intended to maintain the terminally ill patient at home during a pain or symptom crisis. Medicare reimburses this at an hourly rate General Inpatient Care Short-term care provided for pain control or other acute symptom management that cannot feasibly be provided in any other setting. Inpatient Care begins when other efforts to manage symptoms have been ineffective. Inpatient Care can be provided in a Medicare certified hospital, hospice inpatient facility, or nursing facility that has a RN available twenty-four hours a day to provide direct patient care. Home More Support when it Matters Most When a loved one enters their final days, families need more support—not less . At Wings Over Texas Hospice, we increase medical and emotional support visits as needed to help provide comfort, answer questions, and support the patient and those who love them. Our care doesn't end when a loved one passes. Bereavement support remains available to help family members and loved ones navigate the days and months that follow. Medical Support Additional visits and clinical support focused on comfort and changing needs. Emotional Support Extra guidance, listening, and support for patients and families during the final days. What families we serve are saying Robert Lariviere on Google "Wings Over Texas was amazingly compassionate, efficient, caring, knowledgeable, and most importantly... they were honest and up front on what was about to happen to us (as a family) and my wife when we brought her home for home hospice care. This was the hardest thing I and my children had ever had to endure! They were always there for us to answer questions, deliver care to my wife etc... from the delivery of the hospital equipment, to the techs that checked on her daily, and the RN that was monitoring her (my wife) EVERYONE involved with my wife's comfort during her last days with us was just above and beyond. They treated my wife (and us) like we were their own family! I'm very thankful to EVERYONE at Wings Over Texas. " James Hudson on Google " Miranda and the team have been a blessing to us. They served as an awesome extension of our family as we looked after Mom. " Tayla Tidwell on Google " I can’t say enough good things about wings over Texas. April, Miranda and Alyssa were great. Megan was the on-call nurse when my dad passed and she was great too. They made the whole process easier on us. • 2026 Hospice CAHPS Honors Elite Award | Wings Over Texas Hospice

Wings Over Texas Hospice is 1 of 5 in Texas a 2026 Hospice CAHPS Honors Elite, recognized by MatrixCare for excellence in patient and caregiver experiences. View the Winner List Wings Over Texas Hospice Named a 2026 Hospice CAHPS Honors Elite Recipient A Recognition Rooted in the People We Serve Cleburne, Texas | August 14, 2026 Wings Over Texas Hospice is honored to be named a 2026 Hospice CAHPS Honors Elite recipient by MatrixCare, recognizing excellence in patient and caregiver experiences as measured through Hospice CAHPS satisfaction scores. For our team, this recognition is about much more than an award. It is about people. It is about every patient who has trusted us with their care, every family member who has welcomed us into their home, and every caregiver who has relied on our team during one of life's most difficult seasons. It is about the nurse who walks through the door when a family does not know what to do next. The aide who notices a small change that makes a big difference. The chaplain who knows when to speak and when simply to sit beside someone. The social worker who helps a family navigate an overwhelming moment. The volunteer or musician who brings a little joy into an otherwise difficult day. It is a team of real people caring for real people. And that is what makes this recognition so meaningful to Wings Over Texas Hospice. 2026 Hospice CAHPS Honors Elite Recipient | 5-Star Medicare Rating | Texas-Owned & Locally Operated | 24/7 RN Support What the 2026 Hospice CAHPS Honors Elite Award Means The Hospice CAHPS Honors program recognizes hospice agencies that demonstrate strong performance in the experiences of patients and caregivers. The program is based on results from the Hospice Consumer Assessment of Healthcare Providers and Systems (CAHPS) survey and recognizes hospice agencies that work with MatrixCare to administer their Hospice CAHPS surveys. For Wings Over Texas Hospice, the 2026 recognition is especially meaningful because Hospice Honors Elite is a special recognition reserved for hospices that score above the MatrixCare National Performance Score on all 22 evaluated questions. Hospice CAHPS Honors Hospice CAHPS Honors recognizes hospices that scored above the MatrixCare National Score on at least 17 of the 22 evaluated questions. Hospice CAHPS Honors Elite Hospice Honors Elite recognizes hospices that scored above the MatrixCare National Performance Score on all 22 of the 22 evaluated questions. For Wings Over Texas Hospice, that means our performance exceeded the MatrixCare National Performance Score on all 22 evaluated quality indicators during the 2026 program's evaluation period. These numbers represent more than statistics. They represent the experiences of the patients, caregivers, and families who allowed our team into their lives. 22/22 Evaluated Quality Indicators Above the MatrixCare National Performance Score The Hospice CAHPS Honors Elite designation is reserved for hospice agencies whose performance exceeds the MatrixCare National Performance Score on all 22 evaluated questions. Wings Over Texas Hospice earned this Elite recognition based on performance across all 22 evaluated quality indicators. For us, every number represents a person. Every response represents a patient or caregiver experience. Every measure reminds us of the responsibility we have when a family invites us into their lives. How the Hospice CAHPS Honors Elite Award Is Determined The 2026 Hospice CAHPS Honors program evaluated Hospice CAHPS survey results from **April 2025 through December 2025**, a three-quarter evaluation period aligned with the implementation of the revised CAHPS Hospice Survey. To qualify for recognition, a hospice agency was required to: Work with MatrixCare for Hospice CAHPS survey administration. Score above the MatrixCare National Score on the **Willingness to Recommend** question (Q31).* Have at least one completed survey returned in each of the three quarters of the evaluation period. MatrixCare then evaluated hospice performance across 22 quality indicator measures. Individual hospice performance scores were calculated using completed surveys from the evaluation period and compared question-by-question with the MatrixCare National Performance Score calculated from survey data from participating hospices in the MatrixCare Hospice CAHPS database. Honors vs. Honors Elite Hospice CAHPS Honors Recognizes hospices that scored above the MatrixCare National Score on at least 17 of the 22 evaluated questions Hospice CAHPS Honors Elite Recognizes hospices that scored above the MatrixCare National Performance Score on all 22 of the 22 evaluated questions That distinction is why the word "Elite" matters. A Recognition Rooted in the People We Serve Some awards recognize numbers. This one reminds us why the work matters. At Wings Over Texas Hospice, we believe hospice care is about much more than managing symptoms. It is about caring for the whole person and supporting the people who love them. It is about listening. It is about showing up. It is about answering the phone when a family is unsure what to do. It is about helping a patient remain comfortable and surrounded by the people, places, and memories that matter most. Our nurses, aides, social workers, chaplains, volunteers, and support team members each play a different role, but we share one responsibility: To treat every patient like we would treat our own family.
