Lead the clinical care of hospice patients while building meaningful relationships with patients, families, and the care team.

Develop relationships with patients and families while coordinating individualized hospice care with the interdisciplinary team.

You'll use your clinical expertise but being present is just as important

Assess and monitor patient needs
Develop and update individualized plans of care
Provide skilled nursing care
Manage pain and symptoms
Communicate with families, facilities, physicians and members of the interdisciplinary team
Educate patients and caregivers
Coordinate services and supplies
Monitor changes in patient condition
Provide emotional support and reassurance
Document care accurately and timely
Advocate for patients and families
Participate in the hospice plan of care

RN case managers are the clinicians assigned to patients M-F.

We have additional 24-hour on-call nursing support available to patients and caregivers.

Our Granbury team serves patients throughout Granbury, Hood County and surrounding communities.
