To be considered for the Hospice CAHPS Honors Award, the hospice agency must have scored above the HEALTHCAREfirst National Performance Score on the Hospice CAHPS Willingness to Recommend question. If that criteria is met, performance is evaluated utilizing a set of 23 additional quality indicator measures. These scores are then compared on a question-byquestion basis to a national performance score calculated from all partnering hospices contained in HEALTHCAREfirst’s Hospice CAHPS database. Effective pain management and nutritional support are important parts of your treatment. The plan of care includes five basic areas: • Physical care • Psychosocial needs • Spiritual needs • Bereavement care • Personal care and comfort The plan is reviewed and updated as needed, based on your changing needs. All medical information will be provided to assist you in participating in your plan of care. We will compare this list to the medications ordered by the physician. Our staff will continue to compare the list to the medications that are ordered, administered or dispensed to you while under our care so any discrepancies (such as omissions, duplications, potential interactions) can be resolved. Your Wings Over Texas Hospice nurse will also review with you what medications will be paid for by your insurance (i.e. Medicare) benefit. Do-Not-Resuscitate (DNR) Order or Out of Hospital DNR (OOHDNR) or Pre-Hospital Medical Care Directive What is it? A Do-Not-Resuscitate (DNR) Order Out of Hospital DNR (OOHDNR) instructs physicians and all medical institutions who might care for you that in the event of cardiac or respiratory arrest, you do not consent to the administration of cardiopulmonary resuscitation (CPR) in any form. A Pre-Hospital Medical Care Directive is a directive that allows patients to refuse specific life-sustaining treatments outside the hospital. Why would I need a DNR? Why do I need one? A DNR form will tell health care providers not to use CPR and other life-sustaining techniques. If you do not have a DNR Order, health care providers may do everything medically possible to revive you. All of these DNR forms are state-approved. The correct state form must be used to be recognized and valid. If you would like a DNR • Ask your healthcare provider for the DNR Order or Out of Hospital DNR (OOHDNR) or Pre-Hospital Medical Care Directive. • Complete the form. Keep your original form and at least one copy. • Talk with family, clergy, and/or friends. • You can also notify health care providers of your DNR decision by wearing an approved ID necklace or bracelet.
